Tying new foundation to old

I am adding a 12' x 12' addition to my home. I already have the footings poured and will be laying 8" concrete block for the foundation walls. How do I tie in the concrete block foundation to the existing poured concrete walls of my existing house. I was thinking of using angle iron with tapcons holding it all together. Is this feasible?
